1. Simply fill out an entry form and request a Maxi-Aids 2009/10 Catalog (if you don’t already have one)
2. Keep the catalog nearby, especially on drawing days (see schedule below under CONTEST PERIOD)

3. If we pick your name and call you, answer a simple question about the catalog and you’ll win a $1,000 SHOPPING SPREE at MAXI-AIDS!!!

The sponsor of the MAXI-AIDS $1,000 “KEEP THAT CATALOG” CONTEST (the "Contest") is Maxi-Aids, Inc. ("Sponsor"), having a place of business located at 42 Executive Boulevard, Farmingdale, NY 11735.
No purchase is necessary to enter or win. This contest is void where prohibited, taxed or restricted by law. All applicable federal, state, and local laws and regulations apply.
Participation constitutes entrant’s full and unconditional agreement to and acceptance of all Official Contest Rules as detailed below.
HOW TO ENTER:
1. Follow the link to the entry form. You will need to enter your name, along with a valid address, phone number and email address. (Note: All of these fields must be completed for an entry form to be valid.)
2. At this point, you’ll have an opportunity to order a copy of the new 2009/2010 Maxi-Aids Products for Independent Living Catalog if you do not yet have one. Catalogs are available in printed form, on a CD-ROM or at www.MaxiAids.com as an interactive version or as a computer download. It is important that you get this catalog and keep it nearby (see DETERMINATION OF WINNERS below for further details.)
3. One entry per person/e-mail address. Additional entries will be voided. One entry makes you eligible for all future drawings throughout the year (No need to re-enter each month.) An entrant can only win one prize during the Contest Period.
ELIGIBILITY: To be eligible, contestants must be US residents 18 years of age or older as of the date of entry and have a valid e-mail account. (Winners will be required to show a driver’s license or birth certificate as proof of age before being issued a prize.) The contest is also open to government agencies and charitable organizations. Maxi-Aids employees, their immediate families (spouses, parents, siblings and children) and those living in their immediate households, whether or not related, are not eligible to enter or win a prize.
CONTEST PERIOD: Entries will be accepted beginning on January 12, 2009 at 12:00a.m. EST. The Contest will end with the determination of the winner of the final drawing on Friday, December 4, 2009. During the contest period, there will be separate monthly drawings on the first Friday of every month, as follows:

DETERMINATION OF WINNERS:
1. On the dates indicated in the Contest Period section of the rules, contestants for each monthly prize will be chosen at random via a computer-based randomizer.
2. If your name is chosen in one of the monthly drawings, Maxi-Aids will contact you by phone (at the number(s) you provided on your entry form) and you will be asked to look at your copy of the 2009/2010 Maxi-Aids Catalog and answer a question – for example, “What item is at the top of page 33?” You will be given 3 minutes to respond with the correct answer. (Note: If you have ordered the new catalog, but have not yet received it at the time of the phone call, you may be asked a question regarding the Maxi-Aids 2008/09 Catalog.)
3. If you successfully answer the question, you will be the lucky winner of the $1,000 Shopping Spree at MaxiAids.com for that month.
4. If Maxi-Aids is unable to reach you or you are not able to answer the question, an alternate contestant will be chosen. Maxi-Aids will continue to pick names at random until a contestant answers a question correctly.
PRIZES: Each monthly winner of the MAXI-AIDS $1,000 “KEEP THAT CATALOG” CONTEST will receive an e-Gift Card redeemable for a $1,000 Shopping Spree at Maxi-Aids. Winners are responsible for shipping charges and any sales taxes that may apply. The e-Gift Card may be applied to merchandise, sales tax (where applicable) and shipping charges. The e-Gift Card is good for 1 year from the date the prize is awarded and may be used at one time or for multiple visits.
Prizes are non-transferable and non-saleable. All federal, state and local taxes, and any other fees, costs and expenses related to the prize awarded and the Contest are the sole responsibility of the winners. No alternative prize, cash equivalent, or other substitution is permitted except at Sponsor’s sole discretion.
ENTRANT OBLIGATIONS & PRIVACY:
All persons submitting entry forms agree to the Official Contest Rules.
To be declared a winner, a selected participant must: (a) confirm compliance with the Official Contest Rules; (b) accept the prize as awarded; and (c) consent to the use of the participant’s name, city and state of residence, photo and quotes from discussions with Sponsor’s representatives for publicity purposes by Maxi-Aids without further compensation to the winner.
Any personal information provided in order to enter this contest will be used solely by Maxi-Aids and will never be shared with any third party.
GENERAL RULES:
Maxi-Aids is not responsible for: (i) any technical malfunctions/problems with phone lines, computer systems, servers or software; (ii) the failure of any e-mail or entry to be received by Maxi-Aids due to technical problems or traffic congestion on the Internet or at any web site; (iii) any injury or damage to participant's or any other person's computer related to or resulting from participation or downloading any materials in this Contest (iv) any errors, delays or other problems related to requests for catalogs or the mailing of catalogs; (v) any misprints or typographical errors on Maxi-Aids websites or on any materials related to the Contest.
The Contest will be monitored by the accounting firm of JK Pescow & Co., CPA’s.
Maxi-Aids reserves the right in its sole discretion to cancel or suspend the contest should viruses, bugs or other causes beyond the control of Maxi-Aids affect the administration, security, fairness, integrity or proper conduct of the contest.
